Clinical stages of syphilis
Primary -
After exposure with pathogen a papule develop at site of inoculation
Painless firm chancre
Regional lymphadenopathy
Healing occur spontaneously within 2-3weeks.

Treatment
penicillin g Benzathine 2.4million international unit (iu)-gluteal region
Intramuscular 💉
Doxycycline 100mg twice a day for 2 week in penicillin allergic patient.

Secondary -
4-10week after appearance of primary lesions
Constitutional symptoms develope with fever

O/E -
Generalised lymphadenopathy
Generalised skin rash whole body including palm and sole but not face.
Condylomata Lata -watery plaque like lesions found at perianal area and other moist body parts.
Acute neurological sign - aseptic meningitis, cranial nerves palsy

Investigation
Serological test
Cutaneous and mucous membrane also show traponema palladium on darkfield 🔬 microscopy .
There is evidence of hepatitis and nephritis.
Transient CSF pleocytosis seen in 30-70% cases.
Treatment same as primary if there is no involvement of cns.

Tertiary -
This occur any time after secondary

Seen in 1/3 in untreated.

Localised gummatous lesions -
Granulomatous lesions.

Benzyl penicillin 3-4 million units 4 hourly for 10-14 days.
