pleurisy ,causes ,symptoms,Diagnosis,management

pleurisy -refers to inflammation of both layers of pleurae(parietal and visceral)
pleura covering the lung and chest wall
or pain arising from any disease of pleura.

causes -
pneumonia, tuberculosis 
malignancy invading the pleura 
pulmonary infarction 
rheumatoid arthritis & SLE .

symptoms -
chest pain (pleuritic pain)
sharp localized pain made worse on deep inspiration, coughing and bending movement.
intercostal tenderness on palpation 
evidence of infection -fever, malaise, increased WBCs.

Diagnosis -Chest x ray may show pleural thickening  
sputum examination may indicate infectious organism.
examination of pleural fluid obtained by thoracentesis for smear and culture.
pleural biopsy.

on examination -
diminished movement of chest on affected side 
breath sound may be diminished on affected side 
pleural rub -a crackling sound at the end of inspiration and beginning of expiration localized usually to small area of chest wall 

management -
bed rest
treatment of underlying disease 
analgesics -pethidine 50 mg I/M or tab Dolobid BD or tab postan 2 TDS, indomethacin .
SYP.actifed DM for cough three times daily.
