Clinical features -
Tender parotitis -75% bilateral
Fever low grade , malaise
If complication like orchitis, meningitis occur than high grade fever observed.
Usually first one side gland involved and than after 2-3 day other gland involve.
Some time one gland involved after other recovered.

Complication
Orchitis -
Orchitis is an inflammation of the testicles. Both testicles may be affected by orchitis at the same time. However, the symptoms usually appear in just one testicle.
This kind of testicular inflammation is often associated with the mumps virus.
Management -
Scrotal support and ice bags
Incision required in severe caseAnalgesic for pain
1%procaine solution injection 💉 at spermatic cord at external inguinal ring
Hydrocortisone 100mg IV prednisone 20 mg 6 hourly for 2-3days .
Oophritis -
Lower abdomen pain
Ovarian enlargement
Meningitis -30% of cases
With fever, head ache, neck stiffness.
Management -symptomatic treatment
Managed by mannitol and steroids.

Investigation
CP -lymphocytosisSerum amylase -commonly elevated without pancreatitis because here salivary gland involved.
Csf -lymphocytic pleocytosis
Normal to low glucose in meningitis (daily 120g required)
Diagnosis of mumps confirmed by complement fixation test or complement fixing antibody.
