Pelvic inflammatory diseases details -PID
Definition:-the result of ascending infections of genital tract{ vagina {colpitis},cervix{endocervicitis},uterus{endometritis},fallopian tubes{salpingitis},ovaries{oophoritis},pelvic peritonitis,tubo ovarian abscess.
It is common and serious complication of STDs .
Pathway of bacteria:-bacteria moves in female genital tract by semen ,causes the cervicitis –endometritis –salpingitis –oophoritis /tubo –ovarian abscess-peritonitis.
Causes:-STIs –chlamydia and gonorrhea ,it is also occur after following occasion :-
Abortion ,childbirth ,insertion of IUD,endometrial biopsy,surgical transmitted .
Risk factor:-
previous STI,IUCD-INTRA UTERINE CONTRACEPTIVE DEVICE AND IUS-INTRA UTERINE SYSTEM . MULTIPLE SEXUAL PARTNER ,AGE <25 YRS ,YOUNG AGE AT FIRST INTERCOURSE.
Symptoms :-
vaginal discharge ,lower abd.pain ,pelvic pain ,tenderness after menses.
Pain during voiding or defecation.
Others are fever ,general malaise ,anorexia ,nausea ,headache,possibly vomiting ,sx may be acute or severe.
Diagnosis:-
blood :-Full Blood Count {FBC},C-reactive protein TEST [crp]
Microbiology :-bacteria culture
Sonography
Examination:-
general –sign of infection –tachycardia ,fever etc.
Abd-tenderness
Speculum-PV discharge
Vagina-{tubo ovarian abscess}
Management:-
medical :-analgesia ,antibiotics {cephalosporine ,metronidazole } if iucd in situ and no recent unprotected sexual intercourse consider removal.
Surgery –if there is abscess.
Complication :-ectopic pregnancy ,chronic pelvic pain ,tubal infertility.{50%}
